The Team Approach

 

Northland PACE employs a group of professionals called an Inter-Disciplinary Team (IDT) or Care Team that coordinates all aspects of healthcare, some of which may be done in our Day Center Clinic and and some are in-home services for PACE Participants. Our all-inclusive program of care promotes personal attention that helps aging individuals and their families enjoy an enhanced quality of life.

 

The Inter-Disciplinary Team (IDT) Care Team of providers includes:

  • Primary Care Physician

  • Registered Nurse

  • Master's-level Social Worker

  • Physical Therapist

  • Occupational Therapist

  • Recreational Therapist or Activity Director

  • Dietitian

  • PACE Site Director

  • Home Care Coordinator

  • Certified Nursing Assistant (or CNA Supervisor)

  • Transportation Coordinator

 

The IDT (Care Team) creates a plan of care that is individualized for each Participant, and then meets regularly to make adjustments to the plan as necessary. Participants and family members are encouraged to be actively involved in decision making, and are included as team members.

 

As a Participant’s needs change, their care plan will change to meet any new situation. If hospitalization or nursing home placement is required, Northland PACE covers the cost of these services when Participants remain in the PACE program and receive this care within our network of contracted providers.
