The focus of every PACE (Program of All-Inclusive Care for the Elderly) organization, including Northland PACE, is to help adults aged 55+ live in the community for as long as safely possible. PACE is an alternative to Assisted Living and Nursing Home placement. To meet this goal, PACE organizations focus on preventive healthcare.
Northland PACE is the future of healthcare. It utilizes a team-based care approach providing a personalized, comprehensive healthcare. Northland PACE helps aging individuals and their family caregivers by providing and coordinating a full range of healthcare and in-home services. For many elderly individuals, minimal services and/or medications are needed. For others, medical monitoring plus skilled nursing care, therapies, and in-home assistance are required. Northland PACE provides and coordinates all of these services plus health-care appointments and transportation so that participants are as healthy and as independent as possible. In-home services may include personal care (bathing and dressing), nutritional counseling and meal preparation assistance, light housekeeping, and other chores.
Northland PACE is designed to help participants live more independently by offering a wide range of medical and health-related services. The PACE model is most effective when participants actively partner with the team in setting, implementing and ultimately achieving healthcare and wellness goals. We are dedicated to providing a personalized approach to care so participants, families, and Northland PACE health care staff can know each other well and work efficiently together toward achieving shared outcomes.
